Passport Applications can be scheduled by appointment by utilizing the calendar below
The City of Novi offers Passport services for:
· First-time passport applications:
o Adults 18 and older, who have never had a passport before.
o Children 17 and under.
· Lost/stolen/damaged passport applications,
· Applications for those who are past 15 years of issuance of their last passport
IMPORTANT: We are unable to process adult passport renewals if they are within 5 years of their expiration date. We’d be happy to help you navigate how to do your renewal, no appointment necessary.
PASSPORT BOOK VS. PASSPORT CARD
A passport book is valid for land, air and sea travel. The passport card is only valid for land and sea travel between the U.S. and Canada, Mexico, the Caribbean region, and Bermuda
Please Note: Children under age 16 will be able to continue crossing land and sea borders using only a U.S. birth certificate (or other form of U.S. citizenship such as a naturalization certificate.). The original birth certificate or a copy may be used.

WHAT DO YOU NEED TO BRING TO THE APPOINTMENT?
- The applicant! Step one of the process of issuing a passport is verifying the identity of the applicant, so they must be present for their appointment (even if they are a minor).
- Completed DS-11 application. Do not sign until you are asked to do so. Application must be completed in BLACK INK only.
- Valid photo ID (see examples on Get Photo ID for a U.S. Passport).
For minors under 16 both parents/legal guardians will need to present photo ID. For 16- & 17-year-olds, the applicant, as well as one parent/guardian must present photo ID. Those presenting ID for the application must be at the appointment. - Proof of citizenship:
- An ORIGINAL birth certificate with raised seal and filing date. The filing date must be within one year of birth. It should state the date filed or date received by the local registrar and must include both parents’ names. Hospital issued certificates are not acceptable.
- A previous U.S. passport. If the applicant is under 16 years old and has a previous U.S. passport, they are required to submit their original birth certificate in addition to the passport.

An original Certificate of Naturalization. If the child is claiming citizenship under a naturalized parent, the child’s green card must also be provided and submitted.
ORIGINAL PROOF OF CITIZENSHIP MUST BE SUBMITTED WITH THE APPLICATION AND WILL BE RETURNED TO YOU BY THE DEPARTMENT OF STATE.

Proof of parental relationship (minors only)
You must submit a document that lists the parent(s) or legal guardian(s) of the child. Examples include:
a. U.S. birth certificate (also evidence of U.S. citizenship)
b. Foreign birth certificate
c. Adoption decree
d. Divorce or custody decree
e. A court orderSome documents, like a U.S. birth certificate, show both U.S. citizenship and parental relationship. These documents must be originals or certified copies (not photocopies).
If your name is different than the one on the document showing your relationship to your child, submit proof of your legal name change. - One passport photo. Photos can be purchased at the Clerk’s Office for $10 each or you can bring them with you. Please make sure they follow the guidelines listed on U.S. Passport Photos.
Please wear a dark colored shirt in your photo, the background is white, and any light colors will be washed out and impact the photo quality. - Payment:
- Each application must have a separate check/money order payable to the “U.S. Department of State”. See fee chart below.
- Each application will require a $35 acceptance fee to the City of Novi and may be paid in cash, credit card (with a 3% service fee), by money order or check made payable to “City of Novi”.
Payment Instructions:
*The application fee is paid directly to the Department of State via check or money order.
**The Execution/Acceptance fee is paid directly to the acceptance facility.
Two separate payments are required. (Effective 1/23/2023)
